Partnerships with EHRs don't always go very fast, and physicians are not always in the EHR. It's all about workflow, and people aren't always signed in. Vocabulary/Ontology

Shakespeare asks, “What’s in a name? That which we call a rose by any other name would smell as sweet.” But since computers don’t usually smell, the names we use in health care communications are important. Standard vocabularies give us a common name for things, and ontologies tell us how things relate to each other within the underlying domain knowledge. Teqqa’s use of standard vocabularies such as SNOMED, RxNorm, and LOINC to codify all of the data we store gives us the ability to understand and use data from any institution. So when we receive a medication order for ceftriaxone, we also know it is an antibiotic and not an anti-inflammatory. And we know it is a cephalosporin, and that it is a third-generation one.

Care Plans and Order Sets are well established for elective surgery, where what the patient needs is established even before hospitalization, and provider compliance with Care Plans and the subsequent outcomes can be compared. For things that happen during the hospitalization, such as the onset of sepsis, ventilator-acquired pneumonia, severe hypoglycemia, or many other conditions, there have been no ready mechanism for doing so when events occur during hospitalization.

Our Care Plan Goals technology does just that: identifying the onset of such events, tracking if care providers provide the established care, and look at the outcomes.
